MAR Title: An old-maid with a breast cancer

Narration:
2 months ago, I came across with a 42 y/o, female, who consulted with a friend due to a left breast mass of 3 years at the ER.
Apprehensive she seemed, I started by making some gestures that showed compassion while examining her and as a result, she opened up.
On physical examination, the mass was pointing out to a malignancy.
Being an old-maid was the main reason for not consulting, she felt alone and that no one would take care of her. I then told her that this should not be the case and let her family know her condition, to give them a chance to involve themselves. She was then advised admission for work-up and referral to service consultant but refused because she still have to talk to the family.
Last May 12, I was surprised to see her. She have started neo-adjuvant chemotheraphy with enthusiasm and currently on her 2nd cycle. She introduced me to her sisters, who accompanies her every treatment. She hugged me and said thank you.
Insights (discovery, stimulus, REINFORCEMENT):
At a glance, in every patient we see, somehow we already have an idea as to what kind of patient are we dealing with and it is vital that we should establish a rapport to gain their trust.
